Healthcare Hub is part of vaccinatiecentrum.nl. The official Dutch general terms and conditions of vaccinatiecentrum.nl are always leading. In the event of any discrepancy, inconsistency or difference in interpretation between this version and the official Dutch document of vaccinatiecentrum.nl, the Dutch document prevails.
In these general terms and conditions, the following definitions apply:
Healthcare Hub: Healthcare Hub, with its main office at Plantage 8, 6708 WJ Wageningen, the Netherlands, and several branches throughout the Netherlands, registered in the trade register of the chamber of commerce under number 68414943.
Client: the person who enters into an agreement for himself or for a third party with Healthcare Hub.
Customer: the individual to whom the services directly relate.
Care provider: Healthcare Hub or the individual employed by Healthcare Hub.
Products: all physical products, whether or not including advice related to the product concerned.
Services: all activities, including examination and advice, directly relating to the customer with the aim of preventing or treating disease or assessing health status, as well as the supply of items. The services include information, advice, vaccination and, where applicable, the prescription of medication.
Appointment: an appointment for a visit and consultation or treatment at one of the Healthcare Hub locations.
Agreement: the agreement for medical treatment in which Healthcare Hub commits to the client to deliver products or perform services directly relating to the customer.
Price: the cost of the products or services including all additional costs. The price includes VAT unless explicitly stated otherwise.
These general terms and conditions apply to all offers, agreements, deliveries and services and other legal relationships between Healthcare Hub on the one hand and the client and or customer on the other hand. Healthcare Hub provides these in a timely manner.
All offers and quotations from Healthcare Hub are without obligation and do not legally bind Healthcare Hub unless explicitly stated otherwise.
If a client or customer accepts an offer or otherwise places an order, these general terms and conditions apply to the exclusion of any terms and conditions of their own.
The agreement is concluded when the client accepts an offer, makes an appointment via the Healthcare Hub website, by telephone, by email or by any other form of communication, and the appointment has been confirmed. From that moment, a payment obligation arises for the client.
The care provider clearly informs the customer about the intended examination, the proposed treatment, developments regarding the examination and treatment, and the customer’s health status. The same applies to the provision of products.
At the customer’s request, this information is provided concisely in writing.
Actions performed in execution of the agreement require the customer’s consent.
At the client’s request, the care provider records in writing which actions consent has been obtained for and provides a copy.
If the customer is younger than 12 years of age, consent of the parents exercising parental authority or the guardian is required.
If the customer is 12 years of age or older but under 16, consent of the parents exercising parental authority or the guardian is required in addition to the customer’s consent.
In performing the services, the care provider exercises due care and acts in accordance with applicable legal obligations.
Any time indication given by Healthcare Hub for the execution of the services, including for example the receipt of test results, is a target date and not a strict deadline. No rights can be derived from such indication.
If work takes place at a location of the client or customer or another location chosen by them, Healthcare Hub employees and any other persons involved must have unobstructed access. The location must meet minimum comfort and hygiene requirements set by Healthcare Hub, which may be assessed in advance.
The care provider does not provide data to third parties without the customer’s consent.
Unless otherwise provided by law, disclosure of data and access by a third party only takes place insofar as this does not infringe another person’s privacy.
The following are not considered third parties:
• persons directly involved in the execution of the agreement and or the services if access is necessary for their work
• parents exercising parental authority or the guardian, if their consent is required
The customer provides the care provider with all information and full cooperation required for the execution of the agreement and or the performance of the services. Upon request, the customer presents valid proof of identity.
Deliveries and services are subject to the prices stated in the price list. The price list is available on www.healthcarehub.nl.
Healthcare Hub may amend the price list at any time. The price list on the website is not decisive. The price stated in the agreement with the client or customer applies to the delivery of products and or the performance of services.
The client owes Healthcare Hub the price.
If payment after performance of the services has been agreed, the price must be paid within 7 calendar days of the invoice date to a bank account designated by Healthcare Hub.
After payment, the client may seek reimbursement from their health insurance provider. Healthcare Hub does not guarantee reimbursement.
In the event of late payment, the client is in default by operation of law and owes statutory interest from the due date until full payment.
The client also owes all judicial and extrajudicial costs related to the collection of unpaid amounts. Extrajudicial costs amount to at least 20 percent of the outstanding balance.
Healthcare Hub is not liable for shortcomings that cannot reasonably be attributed to it.
This includes, but is not limited to:
• failure of third parties engaged by Healthcare Hub
• strikes or blockades
• war or threat of war
• natural disasters
• serious business disruptions such as fire, cyber attacks, traffic disruptions, epidemics, illness or death of a care provider
Healthcare Hub is only liable towards the customer for direct damage resulting from an attributable failure in the performance of its obligations.
Compensation only takes place if and insofar as the liability is covered by Healthcare Hub’s liability insurance.
If no insurance coverage exists, liability is limited to a maximum of € 1,000.
Healthcare Hub must first be given the opportunity to remedy the failure within a reasonable period.
Liability must be invoked no later than 7 calendar days after the appointment. After that period, the right expires.
Healthcare Hub provides advice to the best of its knowledge and ability and has a best efforts obligation. No specific results are guaranteed.
The client and or customer may cancel the appointment at least 24 hours prior to the appointment.
Cancellations can be made by telephone on working days between 09:00 and 16:00 via 0317-224180.
Appointments not cancelled or cancelled too late are charged in full.
The agreement cannot be terminated by the client if Healthcare Hub has already performed it.
Amendments to these general terms and conditions only apply if agreed in writing between Healthcare Hub and the client and or customer.
If any provision is declared non binding, the remaining provisions remain in force. Parties consult to replace the invalid provision with one that deviates as little as possible from the original intent.
These general terms and conditions, the agreement, the delivery of products or performance of services and all related legal relationships are governed exclusively by Dutch law.
Disputes are first addressed through mutual consultation.
If necessary, disputes are submitted to the competent court in the statutory seat of Healthcare Hub.
